To be accessible to our patients, we participate in many managed care programs. Most of these plans require members to pay an office co-payment for each office visit. Payment of the co-payment will be expected at the time of your visit. We will file a claim to your plan for the balance.
Because these contracts are frequently reviewed and changed, we will ask for a copy of your ID card at each visit. Please contact our practice and/or your health plan prior to your visit to verify our participation in your plan.
If we are unable to verify your insurance information, we require full payment for services rendered at the time of your visit. Thus, it is important for you to provide us with your most recent and correct insurance information at every visit.
We participate in most major insurance plans, including Medicare and Medicaid. For Medicaid, we are currently participating with Amerigroup and PeachState. We are not accepting Wellcare patients at this time.
